Please submit all required documents for COVID-19 claims. Over-the-counter self-administered tests are not accepted unless verified by a healthcare professional. PCR tests administered by verified third parties are accepted as proof.

Claims will be reviewed and adjudicated in accordance with the terms and conditions outlined in your insurance disclosure.
